  1. Development of a Housing Transition Plan:
    • Collaborative creation of a personalized plan outlining the steps and milestones for a successful housing transition.

  1. Assistance with Benefit Applications:
    • Aid in the application process for housing-related benefits, ensuring individuals have access to financial support for affordable housing.

  1. Support in Housing Search and Application:
    • Guidance and assistance in searching for suitable housing and navigating the screening process.
    • Handling the completion of housing applications on behalf of the service recipient, streamlining the administrative process.

  1. Guidance on Budgeting and Lease Negotiation:
    • Financial guidance on budgeting for housing expenses and assistance in negotiating lease terms to ensure fair and sustainable agreements.

  1. Relationship-Building with Landlords:
    • Support and advice on positive relationships with landlords, fostering effective communication, and addressing concerns or issues that may arise.

  1. Transportation and Advocacy:
    • Provision of transportation services to facilitate attendance in housing-related discussions. 
    • Advocating to ensure the individual’s needs are met during housing-related discussions.

  1. Identifying Resources for Financial Support:
    • Assistance in identifying resources for deposits, moving expenses, and essential home goods to alleviate financial barriers to housing transition.

  1. Ensuring Safety and Readiness:
    • Comprehensive assessments and measures to ensure the safety and preparedness of the individual for their new living arrangement.

🏡 Transition Services Budget

 

At 1-Dominion Homehealth Care, we support individuals moving from eligible settings into their own homes or integrated community living. Transition services help cover the essential items and expenses needed to make that move safe, successful, and sustainable.

Clients are eligible for one transition every three years, with up to $3,000 in total support available. These funds are carefully allocated across key areas to ensure the move is complete and comfortable:

 

  • Essential Furniture: Up to $1,000 for items like a bed, table, or dresser
     
  • Household Supplies: Up to $300 for cleaning items, kitchenware, etc.
     
  • Moving Costs & Deposits: Up to $1,700 for movers, application fees, or security deposits
     

Funds can be adjusted between categories, but the total must stay within the $3,000 limit.

We’re here to guide you through every step and ensure you have what you need for a safe and successful move.

Who Qualifies for Transitional Services

 

To receive transitional services through 1-Dominion Homehealth Care, a person must meet all of the following:

  • Be 18 years or older
     
  • Be enrolled (or expected to enroll within 180 days) in one of these programs:
     
    • Brain Injury (BI) Waiver
       
    • Community Access for Disability Inclusion (CADI)
       
    • Community Alternative Care (CAC)
       
    • Developmental Disabilities (DD) Waiver
       
    • Moving Home Minnesota (MHM)
       
  • Be moving from an eligible setting into their own home or integrated community housing
     
  • Be moving to a place that doesn’t already provide these items or services
     
  • Not have access to similar support through other funding sources (like nonprofits or charities)

Where Can You Move From

 

To qualify for transitional services, a person must be moving into their own home or a community-based setting from one of the following places:

  • A foster care home (either family-run or corporate)
     
  • An assisted living facility that offers personal care services
     
  • A hospital stay or medical facility
     
  • A mental health treatment center or institution
     
  • A rehabilitation or behavioral health facility
     
  • A group home for individuals with developmental disabilities
     
  • A nursing home or long-term care facility
     
  • A place that isn’t licensed, like a family member’s house or your previous home

🧾 What Transitional Services Can Help Pay For

 

When you're moving into a new home, transitional services can help cover the costs of getting settled. Support may include:

  • Basic furniture like a bed, mattress, sofa, table, chairs, and dressers
     
  • Everyday household items such as cookware, dishes, utensils, towels, and bedding
     
  • Security and rental deposits required to sign a lease
     
  • Moving-related costs, including truck rental and help with packing or unpacking
     
  • Pest control or allergy treatments to make the home safe and livable
     
  • Utility connection fees and deposits (electricity, water, gas, etc.)
     
  • Window coverings like blinds or curtains for privacy and comfort
