1-Dominion requires a fully executed Service Agreement before any services can be initiated. Communication will be conducted exclusively with the assigned housing and waiver case managers who submit the referral. These case managers are responsible for relaying all necessary updates and service details to the client. Any clients who reach out to 1-Dominion directly will be redirected to their designated waiver case manager.
Referrals that are incomplete or submitted fewer than 10 business days prior to the desired move date may result in service delays or denial.
Purchases will be made only for items explicitly indicated within this referral form. Any item not clearly marked or specified will not be ordered or paid for.
Due to rising labor costs and the essential nature of some services, referrals requiring deposits over $500 and involving all service components (furniture, household supplies, and movers) may face delays and receive limited coordination time. In such cases, priority will be given to processing mover invoices and security deposits first.
The Letter of Guarantee will be issued prior to the move date, while security deposit payments will be processed following the submission of a formal claim.
